I just submitted something quickly late last night, to see how easy the process was and what information could be provided. In all honesty – I rather not share what I submitted as a ‘template’ - If I had more time I’d articulate something better than the few sentences. Also, it didn’t allow me to attach files or provide screenshots as evidence which I thought it would.
The steps I followed:

Clicked Report Misconduct / Report Misconduct Now
In the what problem Drop Down field, I selected “Something Else”
In the what area section, I selected the Shares and Markets
In the markets report, I selected shares traded on stock exchange from the drop-down.
In the what Product, I selected Unusual Buying/Selling
Then I selected Organisation as the Entity, and Yes it is still trading.
I typed AVZ in the Organisation Name and selected it when it appeared.
Then filled in the complaint fields with my take on unusual activity, I wasn’t overly detailed and should have articulated something better.
Complete the remain personal questions
and submit.
You get the generic auto-response saying they will look into it.
