I dont believe that this is uncommon in neuro drugs. Mechanism of action looks at how the drug produces its effect. Trofinetide produces its effect by interacting with the chemicals in the brain rather than binding to a specific enzyme (which would be part of targeted treatment- eg cancer cells). For that reason the mechanism of action is unknown.
Ive heard Steve Davis mention this before and dont believe its a concern at all.
