Sad to say my experience is that many bankers are plain just out of their competency level. You can make a good case for a loan and get away with it based on a good employment record, borrowing money elsewhere and not disclosing etc. The amount of fudging the truth that takes place also with loan apps is quite mindblowing. Not suggesting that the examples discussed on this site are a case in point but many folk are their own worst enemy and when it all fails they blame a banker who often is a person who went from school to be a teller and then progressed and can't tell the right way up on a balance sheet.Ultimately the applicant MUST bear the responsibility one way or the other.
