Backinblack, I think your post is one of the most constructive posts I've read today. You are spot on imo - there should be an acknowledgement by management that there has been a track record of operational problems which are clearly beyond just some unlucky breaks. An RCA should have been conducted by now and some type of corrective action plan to address it - and management should communicate that to shareholders so we know that things will change.
