There are only 3 ways to cause tailings dam failure, 1) poor design (unlikely as GHD is involved), 2) poor construction (even more unlikely since GHD designed and supervised) and 3) poor operational controls. fustbuck correctly points out the dam walls looks very thick (on the old diagrams released by Co it looks like about 80+ metres thick). This issue sounds like a recent management and operational issue in that the flow of the tails was misdirected or critical pumping or discharge procedures were not adhered to. We will find out soon hopefully.
