Trillions of our cells die and get replaced regularly-
when our cells are replaced, does this mean "I" am no longer "me" ?
Yes. For example, many minds cease to know who they are when the wife or lover leaves them or when their physical bodies, due to change in cellular structure, cannot do what they previously used to be able to do.

of course it does. there are physical pills & medications that act upon the brain that will render it unconscious

irrelevant question. the fact is if your sense organs (eyes, ears, nose, etc) are removed or there is brain injury, spheres of consciousness will be lost
What if I lose a leg; will I lose "some" consciousness?
yes. before, when your wife stroked your leg, your mind was conscious of that sensation, which felt pleasant & caused other parts of your mind/body to become aroused. now, after the leg is lost, that pleasant experience is lost forever (apart from a memory) because the neurons in your leg which were linked to the brain via the spinal cord have ceased to exist.
If cells in my hand are self-aware, why am "I" still able to slap myself in the face?

Surely my "self-aware" cells in my hand would refuse to partake in such self-destructive behaviour.
the decision is made by your brain. a trust a lobotomy could cure such self-destructive behaviour
Research on near-death experience suggests consciousness can exist outside the dead body.
the body was never dead, which is why they are called near death experiences rather than total death experiences
the capacities of consciousness are not limited to within the body. for example, eyes , ears & nose can sense objects that are occurring outside of the body
similarly, the mind may have a conscious ability to sense things, via mind, that are occurring outside of its ordinary range
for example, the mind of a mystic living in Bagdad may be able to be conscious of what I am doing in my room in a mosque in Lidcombe
however, this extraordinary consciousness does not imply a consciousness that can function separate from a physical body
