Meerkat, there are clearly set down procedures but you can't outlaw culture. When those at the top are steeped in a culture it is hard/impossible to break. Our Defence Forces and the Catholic Church are finding this, individually [at least publicly] all agree that certain behaviour is "unacceptable" but the culture thrives anyway.
