Where is the equity where some doctors accept the bulk bill amount but have to churn patients to make up their income whereas others opt to charge extra and probably devote more time to each patient.

Many GPs are not as affluent as you make out. Some of the franchise owners for whom the GPs work are the ones probably making a fortune.
