I have now been in IT for way to many years. There has not been a single application that has been launched that didn't have some 'un-catered for user initiated events'.

As an example a system I was in the team of was a $260M system back in the 1990's. Built by one of the big 4, tested by literally 100's of testers, in country and in India was bought to it's knees when a little old lady put a zero in a field she wasn't meant to and a bug allowed it to flow into teh system.

That and several other 'un-catered for user initiated events' meant that the system 'core' was not running completely until 20 days after we started.

Systems have bugs, Agile development attempts top find these bugs early and solve them cheaply and quickly.

You can spend a fortune trying to test for everything.......buts there will be a little old lady out there just waiting to divide you by zero no matter how well you test.
