In the case of Nth Korea they have been making noises and threats for decades without following through, so based on past experience there is probably nothing to worry about.
The US is the bigger worry as they seem to love stirring up wars and invading other countries. And they have been getting away with it because the countries invaded had poor military capability. But Nth Korea with it's nukes and ICBM's has the capacity to fight back and really inflict some damage on the US and/or it's allies.

I don't think the North Koreans can lob a nuke on the US mainland, not at the moment.

And if I was in charge of the US, I wouldn't be waiting for them to gain that capability.

I would do whatever was necessary to stop them.
