I think it is just plain bad luck. Like us all, Tisova was showing all the right signs.
Maybe he took the failing really personally and that is why he is going back to gold. You watch the video's of him talking, this wasnt a guy trying to sell you a used car . . he was personally, mentally invested in this and passionately believed they had found a clearly defined "sitter". . . he might be completely gutted that the stuff above ground didnt match the stuff underground.
He might have left to go back to gold because he deeply was thrown by this whole thing. Ive heard from him in the past personally, he was all in on this. They all were. They would be gutted and wondering what/how Tisova dusted
(And I am saying all this as someone who probably lost more than most when this tanked . . . i have a lot of empathy for the team there . . this wasnt incompetence just plain weird ground)
