Nobody in the scientific community should assume vaccines are perfect, and few would. Research is always a catch up process.
On your perfectly valid point about sanitation etc, quite right. But is that where we should stop?
Measles and whooping cough were horrible in my younger days for instance, and sanitation was pretty much sorted.
