Car parks can be problematic. Perhaps more thought should be given at time of designing them. When reversing out of a spot (also road-side parking) I drive so slow (even if the view is clear) so that if I do hit anyone it won't injure (much).
Pedestrians often are at fault too, for carelessly suddenly appearing on a pedestrian crossing and start walking without bothering to see what's coming.
