isn't it fair to include AFI in your list? Not strictly an index fund but providing similar exposure with exceptionally low management and transaction costs. No trailing comissions so don't expect your high street financial planner to recommend it...
Back to the question at hand,
funds that underperform the index may be badly managed or expensive (in terms of fees)or simply (transiently) out of fashion. I'm not sure AFI is any of these.
