SPP & options, page-6

First time I've participated in a SPP and I noticed this sentence in the prospectus:

2.2 Applications for New Options under the Placement Options Offer may only be submitted by the Placement Subscribers (or their nominees) and must be made using the Placement Options Offer Application Form that accompanies this Prospectus.

Does this mean that I need to submit a form to apply for the free options that come with my allocated shares?  I've already made my BPay payment. 

There was no form accompanying the prospectus - anyone know where I can find it?
