If Xanamem could help with insight as well as cognitive performance, yes that would be very valuable for some. But not for others who are so unwell that arguably lack of insight is a blessing.

My late sister was one of the latter, with chronic paranoid schizophrenia. (She died of an unrelated illness). So it would have to be prescribed on a case by case basis. My sister had very disordered thinking and delusions but perfect memory. If she had been "cured" to the extent of having better insight, self-harm may have been the outcome. She somehow managed her insight privately.

An issue in clinical trials for paranoid schizophrenia will be that patients may be unwilling to take unknown medications. It's hard enough to get them to take known medications, which do have genuine adverse side effects. But not all persons with schizophrenia are paranoid, so I would suggest to start with less severe cases as we are doing for AD.
