You can contact Cluff and ask them to send you the necessary form, or you can try calling Cluff's share registry, which as it happens is called Registries Limited - their number is 9290 9600 - once you fill the form out you send it together with a cheque to Registries Limited before the exercise date, they then bank your cheque, request clearance from Cluff to alott your shares, then assuming it is cleared, they organise for the Annc that you have exercised to be posted with ASX, and they also contact your broker via CHESS (eTrade or Commesec for instance) instructing them to convert your holding from options to the exercised share equivalent - individual exercising of options takes about 5 business days - but when you exercise about the same time everybody is, then it can take up to 10 business days for your options to show as shares in your account with your broker - otherwise once the form and cheque have been sent off its no use trying to follow it up - its a pretty well oiled system - you just have to go with the flow - and how do I know this? I called Cluff then Registries Limited today, cheers
