A general observation would be that when the blame game starts heading in a particular direction, those in its path get sick of it rather quickly.
It is vital that we circle back to what went wrong, at this stage it appears that recommendations form the Victorian RC 2009 were working well until a review in 2015 changed things. Some will misleadingly call this a 'blame game', a pejorative which it is not.
