I think the correct term would be "glacially".
As for the paradigm shift I think it's moving along much as expected with a small hiccup due to COVID but management seem to be moving forward on anything they could (e.g. hernia, practicing hospitals, training) while lesser management might use it as an excuse to pause.
As for decreased infection the other poster can speak for themselves but my interpretation was "decreased infection as compared against other methods" which would render your "nil" comment as missing the point.
