My most recent one was a penny mining stock. They did a 12:1 consolidation. That stock is now trading @ 1.8c and it pushed my cost out to 23c per share. Not even worth selling now.
Another one that springs to mind (i think) was First Cobalt, Consolidation and then delisted last year.
