The orange mask he wears probably keeps his skin a pasty white colour underneath.
I would have to agree that the standard of care does influence outcome. There seems to be a clear difference between outcomes in some hospitals and countries than others.
Having said that, being old, fat and low Vit D levels do seem to be associated with poorer outcomes.
But Boris survived, Trump just might make it also. He's already lived longer than his diet and lifestyle suggests he should.
