I just called Computershare and spoke to a bloke called Iman. He said if I am paying via bpay, I do not need to submit the form. This applies even if I apply for top up shares beyond my entitlement.
So it looks like they divide the amount you pay by 28c to work out how many shares you are applying for. The reference number you provide in Bpay should uniquely identify holders.
