pension lump sum proceedure?

hi
Does anyone know the exact proceedure to pay a lump sum from my smsf income stream?

eg if I wish to pay myself $10k from a $100k income stream as a lump sum (not as a pension)

I gather this is a simple commutation?

which pieces of paper do i need to generate
1)for me?
2)my smsf
3)the ATO?

do you know any links to the web explaining this.?

my administration company says its very difficult.

the generation of the income stream was very simple - just a few minutes and nothing o the ATO
