Three recent studies in the United States have reported positive results for the use of both medicinal cannabis as an  opioid sparing therapy for the treatment of chronic pain and CBD for the management of pain following rotator cuff surgery.


Researchers at Rothman Orthopaedic Institute in Philadelphia conducted a study of 186 patients with chronic back pain and 40 patients with chronic arthritis pain who were using opioids for pain control.
Trial participants were provided access to medicinal cannabis and then had their opioid use tracked using the opioid measurement called morphine milligram equivalents (MME).

For the treatment of OA, patient outcome measures were assessed at 3, 6, and 9 months following medical cannabis use.

A significant decrease was seen in the average MME per day of prescriptions filled by patients, from 18.2 to 9.8. The average drop in MME per day was 46.3%.

The percentage of patients who dropped to zero MME per day was 37.5%.

OA patients' pain scores decreased significantly, from 6.6 (n=36) to 5.0 (n=26) and 5.4 (n=16), at three and six months, respectively.

The back pain patients also reduced their average daily opioid use, from 15.1 to 11 MME.

Patients in both groups reported a reduction in their pain symptoms and an improvement in their physical health.

About 37% of arthritis patients and 38% of back pain patients quit opioid painkillers altogether.

Study results were presented at the recent 2022 Annual Meeting of the American Academy of Orthopaedic Surgeons.

A single-centre prospective study followed a group of 115 patients who were already using opioid therapy for chronic pain and who agreed to trial medical cannabis treatment.

The study was performed at the Allegheny Health Network Institute for Pain Medicine in Pittsburgh, Pennsylvania.

There was a 67.1% average decrease in (daily morphine milligram equivalent) MME/patient from 49.9 to 16.4 MME at the first follow-up. There was a 73.3% decrease in MME at second follow-up (at 6 months) from 49.9 to 13.3 (P < 0.0001).

By the end of the study, 75 of the 115 patients chose to remain certified for medical cannabis as they had significant pain relief and subsequently weaned down on opioids.

Results of the study were published in Pain Physician Journal in January.

A placebo-controlled Phase 1/2 trial with 99 participants studied the effect of a 50mg, fast-acting oral CBD therapy, administered 3x daily, for the management of pain for 14 days after rotator cuff surgery.

The research was conducted by researchers from the Department of Orthopaedic Surgery at NYU Grossman School of Medicine and was presented at the recent 2022 Annual Meeting of the American Academy of Orthopaedic Surgeons.

Patients receiving the CBD reported lower pain and higher satisfaction with pain control than those on placebo, with no major side effects.
