I've spent a fair amount of time looking into the history of Big Bell. After the last fatal incident which occurred over 20 years ago, there were a number of safety studies performed on BB. Some of the reports are still available online if you dig around.
At the conclusion of the study there were a number of safety recommendations. These included various explosives handling procedural adjustments, enforcing a number of mining exclusion zones, increasing number of access points and reinforcing potential mine stress points. There were many remediating actions that have now become commonly used at deeper UG mines that are more prone to seismic activity. The number of deaths caused by rockfalls have decreased substantially worldwide as geological understanding and mining safety technologies have greatly improved. The mine has since operated without any serious incident.
As you said mining is an inherently risky job. It's often the devil you don't know about that causes the most carnage.
