You should get an email from link when the form is up - provided you've got your communcations preferences set. Then you just use the details there to transfer your money (choosing one of the dollar amount options listed in the prospectus). Then after that it's a waiting game until they close the offer, issue the shares (most likely with some scale back, I.e. you won't get all the shares you applied for). Then sometime after the allocations are announced they'll transfer you any refund. I know for some here that refund took quite a while to come through last time, mine was quite quick though.
