Management might be excellent at what they doing like technology trials etc that their expertise.
Pre-commercialisation is not much expensive and not that hard.
Commercialisation is a completely different game, you will need more people and more Money. Hiring the right people starting from HR, IT, Sales and Marketing, and more clinical trials and more validation (Welcome to the Bureaucracy in the health field). That alone is VERY EXPENSIVE and the company will need to raise more money than ever raised pre-commercialisation stage.
