The link under the form you clicked to open is "Transaction fees". If I read it correctly it mentions a MINIMUM of $125 to convert shares (more shares = more $$). But fees aren't explicitly mentioned on the transfer form. That's why I was confused, and didn't get an answer out of dial-a-3rd-party-support.
