Having read the response to Q4 a few times ... I'm no expert, but I think it's saying that:
• we had some results that weren't world-beating, so we forgot about them
• we found a nugget, which made us realise there was a f@#k-up
• we fixed it (bit-late, a year, maybe a bit more)
• we don't admit we did anything wrong
• we never apologise
• are we following the rules? we are now
• next question?
