2/ Chess and DRS Advise
3/ US Computershare Trust Co, 150 Royall St...etc
4/ DRS transfer
5/ Are you affiliated ? Individual ?

They will send you their ACAT PDF, complete and return. Activate E signiture
Make sure the DTC number is 7807.
Deposit IN $150 USD through your portal.
Keep in contact with your rep. He is in the process in doing others so will streamline requirements for you
