Lack of mental health staff and beds are the main problem, although if drugs are involved the issue is much more complex.
Some schizophrenics divulge little of the disturbing thoughts they harbour although this case will be interesting because it turns out he had a fixation with knives - big red flag obviously- and was known to have mental health problems.
This likely could have been prevented if family had reported it and authorities failed to act.
I ve seen this many times over the years with devastating results.

Welfare and health systems all over Oz are in a state which is severely lacking

I’ve seen a dreadful lack of support for families and individuals.
