Insurance works collectively on odds, not just individually.
Say BOT get 10% ie 370,000 regular patients, and as you said the price is the entire insurance contribution.
A. 370,000 people represents 0.1% of the population, so the other 99.9% covers the profit. Odds are the insurer can insure another 1000 people, and 1 will need this drug.
B. If the insurer offers more coverage, they are more likely to have more people to insure, and higher margins.
B. If people stop taking something else to take sofdra, then presumably there is the reduction of cessation of the other product too.
C. If the insurers stop offering drugs because they are expensive, then there is no point being insured in the first place.
It's far more complicated than what I mention, but I have not seen any evidence of insurers pulling products that people are all wanting to use. ( then they move to another insurer? ) If you can find an example, that may be more related to your concern.
