The ARDS trial was terminated early. Part of the problem was significant changes to SoC while the trial was underway. Initially, patients were not treated with steroids, reversing previous ARDS protocols (out of fear steroids might worsen a viral infection). Then, overnight and about halfway through the trial, steroids were added back in (comparable to treatment of other causes of ARDS). There were significant changes in the criteria for intubation as the trial progressed, leading to the selection of significantly older patients with extra-pulmonary disease. Nevertheless, there were indications of effectiveness and even a hint of synergy with steroids. More study needed, search for Vanderbilt cooperatve trial if you're interested.
