The therapeutic question can't be answered with this Dengue model. Because the disease is mild with the weakened strain the outcome is the same no matter what ISLA-101 does - if the volunteers already have mild symptoms before treatment, and they only have mild symptoms with no treatment, then there is nothing to measure (mild verse mild). ISLA-101 could stop viral replication instantly and the outcome of the study would be the same as if it was just a sugar pill.
Therapeutic outcomes for Dengue can only be answered in the field with patients who are infected with one of the dangerous wild type strains. Such a field trial would look for lower rates of serious disease in those treated with ISLA-101 post-infection.
If you can change the disease seriousness post-infection is unknown. If I had to bet, the answer is it is likely that ISLA-101 will have some effect on the serious disease rate, but in practice that treatment will start too late for most patients if you wait until Dengue symptoms to arise.
The main commercial market for ISLA-101 is as a dengue preventative. I don't know what sort of field trial ILA are planning, but I imagine that they would be looking at giving ISLA-101 as a preventive in patients at high risk of getting infected and developing serious dengue. The results of the 2a trial suggests that there is a excellent chance that ISLA-101 will work as a preventative. I look forward to learning more about the next trial.
