The J&J field trial for the drug they pulled the plug on is probably a good design. What they did was give the drug to the family of an index infection. With this design if a child in the family came down with Dengue then they provide the drug (or placebo) to everyone in the family and looked to see if they prevent or reduce additional cases of Dengue within the family. This is quite a clever approach because the family is highly motivated having just seen what Dengue does one of their own and also at high risk of infection because of their circumstance.
