i suspect that the above features are probably more important than anything else. If heroin were the same, cheap, readily available(on prescription) and accurately dosed then there probably wouldn't be much of a drug problem.
Britain had a system like that until the early 90's(I seem to recall) worked well, so well in fact they stopped it and reverted to street distribution. More bottom shelf thinking.
