I think it's just the one form called the "Entitlement, Acceptance and Application for Additional Shares" Form and longwindingrd pretty much explains it correctly.

Whether it's a cheque or Bpay, just send in the total amount you want shares for:
EG If you're entitled to 200,000 shares you would forward/Bpay $3,400. If you want to apply for a further 100,000 from any shortfall then you just send one cheque or Bpay $5,100.
Keep in mind too, that if the shortfall is oversubscribed then it may be allocated on a pro-rata basis. For example, if there are shortfall subscriptions for 60,000,000 shares and there only ends up being 30,000,000 shortfall shares on offer then there will be a 50% pro rata allocation and you will only get 50,000 of the 100,000 you applied for and will get a refund for the balance ($700).
I have a feeling that there will be no shortfall for this issue but I'll still be applying for double my entitlement...just in case.
