Start the SMSF first
I did this with eSuper.
Once done you can download a rollover form from the tax office or for that matter from eSuper. Fill it in and post it to your superfund - the instructions in the form are self explanatory and its quite straight forward.
Some Super funds prefer you fill in THEIR form so also call them and ask if they have a rollover form for trasnferring to your smsf. I recommend you fill in theirs as well as the standard ATO one and send both if they are slightly different. That way you are sure
