This seems to be a common oversight.
The disaster at the nuclear facility in Japan was a result of a power failure, causing loss of cooling to the used fuel storage pools. The tsunami height was above the diesel generator air intake levels, causing them all to ingest water when they were running, instantly destroying them, killing all emergency power at the facility, which lead to a terrible mess, and it just a power failure!
All it would have taken was long snorkels on the intakes to avoid that whole mess, just like a UPS on critical pumps could have avoided this mess and injuries.
Loss of power and its safety ramifications seems to be a missing link in industrial safety evaluations all over the place.
