Dr Who,
By now most things would have been checked and double-checked, eh?
And all straws dangling around would have been grasped?
But what if you have no pressure in your "rail", cause most fuel is just returning to tank? Your pump then would not be able to supply the required pressure (or flow) to the injectors.
Dunno about your Hilux system specifically, but there should be in the system a "Rail Pressure Limiter Valve", and a "Fuel Rail Pressure Control Valve Solenoid".
The failure of either of these, in my opinion, could cause the fuel to return to tank with no pressure.?????
