Trader, your post is showing us a form, one that is computer generated and saved as a file. Who makes amendments to an official form using a pen and then submits it to the State Recording office? This is a joke. Give us all a link as to where this file can be viewed and stop posting you private works. This form shows that the well was abandoned due to a lost hole. If the hole was ruined during the accident, ie.. like a bit bucking and losing a string downhole, that would all make sense. So I am going to consider your post suspect and doubtful in its authenticity.
