Accidents are usually the result of carelessness, stupidity or maybe equipment failure.
The brakes weren't applied, or perhaps failed.
I'm not saying it wasn't a setup, (I no little about mining trucks) but just seems very unlikely given there were people, buildings, and cars etc downhill.
