Andy,
If you go back to your own figures you can see that the packer has happily gone down 434metres into the 5 1/2" pipe so odds are that it is not the wrong packer.
It is hitting something 10 metres into the fracced zone.
To seal the zone they will have pumped concrete into the fracc perforations ... perhaps the pressure has caused one of these plugs to impinge into the pipe zone or something .....
It doesn't have to be someone's fault.

Chopper pilot huh .... always wanted to go for a lesson in one of those egg beater thingy's .... ever since Mash.

There is also one other point .... if you were at the controls of the A380 I would be confident that you would be working your arse off to fly the thing because your life would depend on you getting it on the ground safely also .... lol
