From some very limited reading/research I have done since the announcement on 20 nov, it seems that there may have never been a "pilot plant" taking the process from mined ore at the front end to 68%fe magnetite AND dry tailings at the back end.
It seems that everyone may have been in the dark - they had a plant designed on the basis of a pilot plant which did produced 68%fe, which ticked the box and stuck a tailings system on with appropriate specs., which also ticked the box. Disaster happened when they pressed the go button, don't believe the management had any idea either - not an excuse just seems how it was.
