Depends on the structural damage you have...
I had really, really bad back pain for many years and the best advice I ever got from a doctor was that as long as I could still keep moving and functioning, generally stay away from doctors who propose cutting you open to fix your back since they almost always make things worse.
Since the depths of my back pain and sciatica some years ago, I started on an regular exercise program that essentially stopped the back pain and any problems...
Of course it depends on what sort of back damage you have, but the body is a quite effective healing machine in most cases as long as you do the right things with diet and activity to assist the healing...
