I will kick this of, but let's keep it objective, at the end we we can collate a definitive list.
1. Changed production time line 3 times.
2. Spent a load of debt before securing equity.
3. Starting engineering and site works before approvals and permitting.
4. Set up a a huge overhead before a green light.
5. Kicked off a newsletter.
6. Employed a coo and CFO well ahead when required.
7. Procured a deal with the First Nations whereby they get paid on permitting (not production or subject to funding)
8. Poor cash flow and cost control management
9. Poor investor relations
all IMO .... Keep the list going....
