I would be worried about moles even in the relatively young. Our son had a prominent mole on his arm which was exposed to the sun even when wearing a work shirt.(short sleeve)
Our GP removed it and it was turning cancerous. Thankfully he got clear margins and there have been no further problems. Our son was 19 or 20 at the time. Once he started working for a large resource company they have standard policy of long sleeves and not permitted to roll them up so his sun exposure is minimal.
I would add that our kids were raised to never be outside without a hat and sunscreen and rash vest if in the water.
We have had good results from a yearly consultation with a skin specialist. Never tried the clinics.
