I think may be it is generally hard to distinguish, hence they need the packaging to tell which side is smooth.
A bit like the 3M sticky that tell you which side to strip off first.
May be the package just doesn't have the right instruction and surgeon just assume by looking at the patch (but it is hard, hence they got it wrong).
