Trafford Care Coordination Centre, built on Orion Health technology, set to
go live
In July this year, Orion Health signed a contract with Computer Sciences
Corporation (CSC) that will support the groundbreaking Trafford Care
Co-ordination Centre (TCCC) on behalf of NHS Trafford Clinical Commissioning
Group, in Trafford, Greater Manchester, England. The project is set to go
live.
Under a CSC primed contract, Orion Health technology will be used to develop
an integrated digital care record that will bring key parts of the patient
clinical record together, creating a single view accessed via Orion Health
Clinical and Patient Portals that support both clinicians and caregivers to
make informed decisions on the best course of action for patients.
The aim of the Trafford Care Co-ordination Centre is to revolutionise the way
its local residents are cared for. The TCCC will provide a single point of
contact for patients, their families and health professionals. It will
operate like an air traffic control system, tracking patients as they move
through the health and social care system, guiding them to different
services.
The patients' care will begin with their GP making a referral which will
result in their onward journey being mapped out on a patient care pathway.
Pathways will be based on a localised Map of Medicine - a clinical tool that
enables clinicians to plan care according to the best available evidence.
Once patients are assigned to a particular pathway, their ongoing management
will be overseen by a care coordinator who will be responsible ensuring all
diagnostics are undertaken and for arranging all their outpatient
appointments, social care support and transport to and from hospital.
Gina Lawrence, chief operating officer of the Trafford CCG, says that
Trafford is a financially constrained health economy and for the last few
years has been developing community services that could deliver better
outcomes for patients and better value for money.
"We developed a whole series of community integrated services that work very
well," she says. "But we found that the more things we introduced, the harder
it was to navigate around the system. People were becoming confused about
what was where."
Colin Henderson, Orion Health's general manager in the UK and Ireland, says:

that the right services are made available to patients at the right time? It
means putting the patient at the heart of the system and sharing information.
