Just ring them.
When you speak to someone, after identifying yourself, just ask them to also email you a record of your holdings. That will then serve as proof that the ownership of your shares are now known and the shares cannot be classified as unclaimed.
Re - the W8BEN form, no urgency unless/when you decide to sell them.
